Zero-Incident Safety Protocols: A Case Study
In September 2024, Selective Marine reached a landmark milestone: 1,000 consecutive days without a lost-time incident (LTI) across our managed fleet. This case study examines the systems, culture, and operational discipline that made this achievement possible.
The Framework
Our zero-incident programme is built on three pillars: behavioural safety observation, hazard identification loops, and leadership accountability at every tier.
- Daily toolbox talks — every shift begins with a structured 10-minute safety briefing tailored to the day's tasks
- STOP Work Authority — any crew member, regardless of rank, has unconditional authority to halt operations for safety concerns without consequence
- Near-miss reporting — a no-blame digital reporting system resulted in a 340% increase in near-miss submissions, enabling proactive risk elimination
Results
The data speaks clearly. Since implementing the programme in 2021, our Total Recordable Incident Rate (TRIR) has fallen from 1.8 to 0.3 — well below the industry average of 0.9. Our goal remains absolute: zero harm to people, assets, and the environment.
